You are eligible with a BMI of 27 or higher with a weight-related condition, or a BMI of 30 or higher. If you have previously received a prescription for GLP-1 medication, you may also be eligible in some cases with a lower BMI.

Between 18 and 75 years old

The program is available for adults between the ages of 18 and 75.
